Cabinet Office forms

The Cabinet Office provides on request copies of the forms required to accompany Cabinet papers. Ministerial offices may make additional copies of the following forms, or download them. They are available in both Adobe Acrobat (PDF) and Microsoft Word (DOC) formats:

• CAB 50 candidate information form: (pdf) (doc)
• CAB 51 committee information form: (pdf) (doc)
• CAB 100 consultation form: (pdf) (doc)
• CAB 101 coversheet for submissions form: (pdf) (doc)

The CAB 50 and CAB 51 forms are attached to appointment papers. 

A CAB 100 form must be filled in and attached to all papers that are submitted to Cabinet or Cabinet committees. 

The CAB 101 coversheet is filled in by the Minister's Office and attached to papers when they are submitted to Cabinet Office.
